Can you provide a W-9 and Certificate of Insurance?
Yes. A current W-9 and a Certificate of Insurance (general and professional liability) are included in the Procurement & Security PDF and can be re-issued with your company named as certificate holder on request.
What are your payment terms, and do you take a PO?
Both. You can pay by card at checkout, or request a NET-30 invoice with a purchase order number. Invoices are issued through Stripe Invoicing and bill straight to your accounts-payable team.
Is there a per-employee or per-seat fee?
No. Pricing is a flat rate per live session for up to 200 attendees. There is no per-employee fee, no platform subscription per head, and no metered usage, the price you approve is the price you pay.
Can you complete our vendor onboarding or security review?
Yes. We are a fully insured, women-led small business and onboard into standard vendor and supplier-diversity systems. Sessions require no app install and collect no employee health data, which keeps most security reviews short.
Do participants sign waivers, and is this medical care?
Chakra Hours is educational wellbeing programming, not medical or clinical care. Activity is gentle and optional, covered by our insurance and a standard assumption-of-risk clause in the MSA, so there is no separate per-participant waiver to chase.
How fast can we get a program approved and scheduled?
Most teams go from packet to first session in about two weeks: we return scope and pricing within one business day, the MSA is short, and once it is countersigned and a date is chosen we confirm your facilitator.
